Level 138 is the 139th level of the Backrooms.

Why are you here?

Your throat becomes dry; your forehead and palms begin to drip with sweat.

Be at peace, Gary... Level 138 resembles the inside of a luxurious psychiatrist residential hospital consisting of several areas, some of which will be discussed in further detail later.

You, Gary, nervously watch as this 'entity' resembling that crazed game show host from Level 511 pat their bruised but healing nose.

They looked slightly annoyed by your assault but are calm like stagnant water: Welcome. You are safe here! If you appear on Level 138, you are on the verge of going insane...

You, Gary, tense up as they drift past you like a gliding dove.

Or... you have already gone mad.

You swivel around to not lose track of them.

Everyone here is a little mad.

You wait for them to drop the facade.

The 'entity' sighs with pity and confesses: I am Kill Streak Kim's twin brother. I suppose... they did not tell you they have a sibling.

Your eyes narrow with suspicion. You are not convinced. Even if Kim has a sibling, they could be as twisted and sadistic as they are. This must be a trick! There is a catch. There is always a catch...

Is this 'entity' reading your mind?

You do not have to trust me. I understand this is a lot to take in. The negatives are not quite like the positives. They drift into a philosophical tangent. The madness induced by the positives are created by the reality looking familiar, but there is something uncanny about those places. It might be the physics... or the geometry... that you constantly feel the fabric of the unknown brushing against your cheek. You are consumed by isolation and plagued by whatever memories you are imposing on that space. Your mind is deprived of comfort... support... security... so, your mind fills in that emptiness with unseen looming threats that may or may not be there... The fear... the loneliness... the despair... the longing to return home... and the growing realization that you might never go home is all contributing to your mental decline.

You let your guard down... a little.

I am getting off topic.

You ease yourself down on one of the benches in the lobby. You look up at them with some lingering fear but growing awe and curiosity.

This being does not take advantage of your submission as they carry on: Your condition is quite common here, I am afraid.

If you are showing any mental health issues, emotional instabilities, cognitive impairments, or behavioral concerns (listed below), here is where you need to be…

  1. Survivor’s guilt. You witnessed a good friend getting mauled by dozens of Hounds. You can still hear the screams. Blaming and punishing yourself for their passing will do you no good; it was not your fault.
  2. Prolonged Bereavement. One of your friends have not returned. They are one with the trees. I know as a therapist I am not supposed to judge ... but why go looking for trouble. I digress and apologize for my rudeness... Grief is the normal progression of loss however, it is not good for your mind, body, or soul to allow it to linger.
  3. Depression. Did you escape The Whisper? Say no more.
  4. Anxiety. You escaped.... Level 318? Hmm... when did they change that reality's designation? Anyways... You know you will never be sent back; but still, you have nightmares.
  5. PTSD. You escaped Level 511! It pains me to know that my sibling is one of the reasons why Level 138 exists…
  6. Psychosis. As a M.E.G. operative, you have seen too much sh*t back here… ***sigh*** I know you do not believe me when I say this, but I do not know how to defeat Kim… they are everywhere and nowhere at all… I would advise you to leave that demon be. Tormenting you in their playground is the least they will you to do. If I could stop them, I would have already.
  7. Phobias. Level 7 and Level 8 tap into deep-rooted primordial fears you creatures harbor.
  8. Anger. You were left to die in Level 6 but survived. You want revenge! You should listen to that |faceling officer: revenge is a dish best served cold.
  9. Memory Loss. There are things in here you might want to forget, but you insist on trying to recall what your mind is repressing.
  10. Performance Issues. You write sh*tty wiki pages… this one very well could be one of them… thus, you and I need to talk to someone about it. This is not the place for that. We could consult the mods of this wiki for more insight and guidance, OR, you are struggling to perform daily activities, in which THIS TIME I will be able to accept you here.
  11. Relationship Issues. You need a shoulder to cry because you want certain entities to like you. Are you struggling to make friends with other humans?
  12. Addiction. You insist these cravings are minor. You tell everyone that you can stop drinking Almond Water anytime you wish. You are the only person I have encountered addicted to Almond Water.
  13. Adjustment Issues. You are a Denier/Regretter from Level 4. Acceptance is key, but there is nothing wrong with hope.
  14. Severe Derealisation. What the hell did Cinder do this time? Oh, have you been a victim of Level 602...? Come here. This will be a pain to cure...

Description

Level 138’s psychiatrist residential hospital consists of several areas: The Lobby, Residential Living Quarters, Cafeteria, Therapy Rooms, Recreation Rooms, Medicine Counter, and the Offices. Each room generates an arbitrary tranquilizing effect from an unknown source. Wanderers admitted or leaving the facility have reported feeling safe and secure for the first time in forever. Most wanderers do not want to leave this place. But at some point, they must. Some challenges in life cannot be avoided. Furthermore, this level is dangerous. Staying here too long will cause you to dissociate mentally and physically. You will be scattered across the ethereal plain. You will cease to 'be' but not cease to exist. With that said, you must face the reality of your situation... with pride and dignity.

The Lobby

Admitted wanderers begin their journey towards recovery in The Lobby. This space consists of a circular admissions/exit desk and a hallway leading into other therapeutic rooms. You will be greeted by three |faceling receptionists: Faith, Hope, and Charity. Relax! You do not have to fill out any paperwork or answer invasive questions. We know why you are here. I will have one of the girls escort you to the residential living quarters to get you settled in.

The Residential Living Quarters

Wanderers are given a private room. This space consists of a full-size bed, shelves, dresser, small dining table, and bathroom. You can have guests over in your space if you wish. You can have your meals in the cafeteria or have them brought to your room. Meals are served three times per day in this level. There is no menu. Request whatever pleases you and, the kitchen hands will deliver.

Cafeteria

Wanderers will be encouraged to eat in the cafeteria. Socialising and eating with others is pivotal to human culture. Eating together gives wanderers a sense of community and familiarity. This space resembles your typical group dining hall. The special feature of this room is that it generates a gastrointestinal agent that aids with the digestion of food; releasing oxytocin to increase social bonds.

Therapy Rooms

Wanderers will be encouraged to participate in therapeutic sessions. Sessions can be typical or a blend of atypical treatments. Some of the sessions offered include by not limited to:

  • Sensory Deprivation Series: You in a Void. You will not be sent to The Void.
  • Purple Almond Waterfall guided meditation.
  • Purple Almond Water therapeutic hot spring cleanse.
  • Ethereal Sky floating meditation.
  • Red spiral out of the abyss walking meditation.
  • Dark Kundalini Yoga with friendly smilers.
  • Rage Therapy. Anger is healthy… Let it out! Punch walls. Break Chairs. Dive onto the tables. Smash mirrors and dishes. Charge in the doors. All of these materials will be provided. Scream into a void. No judgment here. I have witnessed and experienced every form of human coping.
  • Almond Cake Therapy. Your choice of Almond Cake will be provided for you to indulge and savor.
  • Talk Therapy. This is the only normal thing that is on this list.
  • Realization Therapy. I will show pictures to you side by side, and all you have to do is spot the differences. Sound simple enough?
  • Touch Grass Therapy. Humor is the best medicine.
  • Darkness Therapy. This is not for the faint of heart...are you sure?
  • Electroconvulsive Therapy...!? He narrows his eyes before confirming. Yes... I know how to administer it... but, I am under the impression you are not seeking treatment... you are seeking information... correct?

The Recreation Rooms

These spaces are empty until entered. These rooms function as an alternative reality that is influenced by the thoughts imposed on them. The space will fill with whatever the wanderer(s) wish to be there. Wanderers are encouraged to dream big and positively, and these rooms will do the rest.


Medicine Counters

These stations are headed and managed by our Nurse practitioner, Maple, Nurse practitioner assistant, Willow, and intern, Ash. You will be prescribed any of the following: Almond Water, and Memory Juice. Before you leave this level, please, take as much as you wish. The medicine counter refills itself.


Dr. Astrid F. Nightingale

Dr. Astrid F. Nightingale manages the treatment center on Level 138. Yes! He is the identical twin sibling of game show host of Escape Rooms: Forget Me Not! Kill streak Kim. (Astrid is not proud to admit his familial bond with Kim.) Astrid is androgynous but identifies as male. He looks human but is angelic in appearance. He is possibly older and more tired than he appears, but he looks 20. Like his twin, he does not show up on any form of camera or video-recorded generated images. He does not have a reflection in the mirror either. Astrid is an entity of order. He is calm, reserved and patient. He is empathic and will encourage wanderers to empower themselves despite their trauma and situation. He does have moments of frustration and compassion fatigue when dealing with difficult "clients".

He diagnosed his twin as a psychopath. He does not mind discussing his brother during a therapeutic session as you are possibly at the clinic because of them. However, if you probe Astrid for information to determine how to defeat Kim and destroy their facility, Astrid will insist he does not know. Despite what members at M.E.G. might think, he DOES NOT know.

You know Kim is not their real name. Their real name is Ashley. Do not call them by their real name. Speak of the devil and he shall appear, speak of my brother and you won't regret your decision for long. They would leave no trace of you but the stench of decay. You can say their real name here. They will not come here because they do not want me to see them. Ashley loves and hates me. Ashley is ashamed and unremorseful. You must understand that my brother is a conflicting and chaotic entity.

Do I love my brother? I love them but not the monster they have become…

Employees

The employees are all facelings. Those were employed by Dr. Nightingale to help run the facility. They fill the roles of receptionists, nurse practitioners, interns, security officers, cafeteria and custodial staff, and recreation room monitors. Their job is to ensure you are comfortable and secure during your stay. Do let them know how else they can assist you in your recovery.

[Temporary] Colonies and Outposts

Dr. Astrid F. Nightingale is allowing temporary colonies to be stationed on Level 138. He is discouraging anyone from staying in Level 138 for more than three years. He and his staff are monitoring progress toward mental recovery and for signs of dissociation. Any individual who stays beyond the 3-year mark begins to dematerialize.

Cuckoo's Nest

  • This group originally consisted of 8 male wardens who are recovering M.E.G. operatives who escaped Level 511.
  • 3 out of these 8 male wardens recovered and have since departed Level 138. I am under the impression the remaining 5 are staying here because they have questions for me.
  • These men collectively are recovering from either PTSD, generalized anxiety, survivor's guilt, memory loss or mild psychosis.
  • They still respond violently to any form of game show background music.
  • They are wary of Dr. Astrid F. Nightingale but admit he is a good entity.

Trail blazers anonymous (T.B.A.)

  • This group consists of 18 wardens (4 females and 14 males) who were survivors of Level 318.
  • 5 members (2 females and 3 males) of the original colony departed Level 138 March 03, 2022.
  • These individuals collectively suffered from either PTSD or generalized anxiety.
  • They are encouraged to do some meditation and yoga to slow down their mind and body.

Forsaken

  • This group consists of 37 wardens (20 females and 17 males) who were survivors of Level 6.
  • These individuals collectively suffer from either PTSD, generalized anxiety, intense nyctophobia, anger, survivor's guilt, or moderate to strong psychosis.
  • They do not like to be left alone. They cannot tolerate the dark.

Tea Partyers' Remorse

  • This group consists of 7 wardens (6 females and 1 male) who were survivors of Level 532. I see you have met my nephew, Tuwile. I am surprised he leaves anyone alive.
  • These individuals collectively suffer from either PTSD, generalized anxiety, survivor's guilt, depression, chronic fear, or strong to severe psychosis.
  • They do not like to be left alone. The Native entity named Amygdala is not permitted around these individuals.

[Abandoned] Colonies and Outposts

Separation Anxiety Anarchist [Recovered and Departed]

  • This group formerly consisted of 7 wardens (2 females and 5 males) who were former colonists of The Deniers/The Regretters from Level 4.
  • These individuals collectively suffered from either generalized anxiety, depression, survivor's guilt, or had adjustment issues.
  • They were eventually allowed to use the recreation rooms as they accepted their situation and was able to move on.
  • All 'colonist' departed Level 138 April 19, 2022.

Entrances and Exits

Entrances

  • You are a survivor of any level with the survival or environmental classification of 4 and 5 and you are experiencing psychological and emotional trauma.
  • Punching a wall at any level has a 1.00% chance of bringing you here. Screaming profane language while punching the wall will increase your chance of entry by 15.00%. (Note: this is a one time chance per level; repeated punches will have no effect regardless of success or failure.)
  • Oh...? Cinder did not tell you... if you select an equal number of light and dark spaces in Level 602... you might end up here. Why? Technically, I am a chaotic entity with good, yet questionable intentions and Level 138 is neither a light or dark space.
  • Buy a chewed up pen in Level 849 to open a door to this place.

Exits

  • Positive Ultra: Achieving a sense of enlightenment and/or inner peace will transport you to a level you feel comfortable in.
  • Positive: Exiting through The Lobby of Level 138 will lead you back to Level 2, 4 or 11.
  • Bad: Noclipping through the walls will result in you ending up in a level with a survival classification of 3 or higher. We have a Lobby...
  • Awful: Failure to comply with staff will result in you ending up in a level with a survival classification of 4 or higher. Or... you could behave and leave out The Lobby...
  • Abyssal: Refusing to leave, disrupting therapy sessions, or being otherwise uncooperative will transport you to a level with a survival classification of 5. Please! Leave through The Lobby if you are going to act up...! *****long sigh...leaves for 30 mins to complete some Rage Therapy then comes back...long sigh***** I am sorry... ... ... We are trying to help but you need to be considerate of others. It is fine if you do not want help. You have the right to refuse help, but you cannot stay here! If you decide to depart, I rather you leave through The Lobby as the other options are much worse. Just do not complain to me when you are worse off. I did try to warn you...
