Level 191 resembles an out of order industrial factory that compose of 2 floors. The only source of light are the LED lamps hanging from the ceiling of the first floor. There will be occasional blackouts can last about 2 to 3 hours. Similar to Level 6 the blackouts can cause a person to lose his/her sanity. Oddly enough, there are no Insanities found in Level 191, suggesting that anyone who has entered this level hasn't gone insane yet.
The 1st floor is composed of many conveyer belts all leading to vents. One of the vents lead to Level 190. There are many crumpled sheets of paper, plastic and cardboard boxes on the first floor. Rarely, weapons are found on the floor of the first floor. These can vary to Baseball Bats to Desert Eagles. There is also a kitchen located beneath the stairs on the first floor. The refrigerator contains Almond Water, but it is not recommended to drink it. There are metallic stairs leading to the 2nd floor. All attempts leaving this level through all vents have failed due to all vents hitting a dead end.
The 2nd Floor is similar to the composition of the first floor. There are numerous random junk on the floors as well as weapons. Though, this floor lacks conveyer belts and vents. There are over 15 doors in the 2nd floor, but only 3 have been confirmed to lead somewhere. These rooms sometimes contain weapons, batteries and flashlights. The other doors in the second floor lead to rooms that are infested with Skin Stealers.
Surprisingly, radios and other devices that are used to contact the outside world, work perfectly in this level. It is advised to use this advantage to contact people if you are ever stuck in this level.
Colonies and Outposts
There is currently 1 known Outpost that resides in Level 191. But, as confirmed by the founder, the Outpost will move once they've found a way to escape this level.

Entities
Known entities that reside in Level 191 include Skin Stealers, Hounds, Smilers and Deathmoths. The Skin Stealers usually stay in the 2nd floor, but rarely, they wander down to the 1st floor.
Entrances and Exits
Entrances
- Entering a broken vent located in Level 190.
- Falling asleep in Level 61 will lead you waking up in Level 191.
Exits
- Discovered by one of the members of The Refugees, it is possible to escape this level by entering a door labelled "Maintenance Facility" then non-clipping through the wall at the back-most of the room. Though this is difficult to pull of, successfully doing this may lead you to either Level 6 or Level 444.
