Level 518: Your Local Pharmacy

This level resembles a CVS Pharmacy location located in a building that seems to be from the mid 2000s. When you first enter, you'll see a prescription counter area that has nothing other than the counter itself, and four chairs. Inside this level are 50 isles which all have a length of 500-1,000 miles, depending on if you have good luck or bad luck. If you walk down these isles, you'll see shelves which all different kinds of medication, made in the form of pills or liquid. However, it is strongly recommended that you don't take these medications, as they are expired. If you take them, you'll become really sick and die within 30 days.

In some areas of this level, you find Almond Water, bags of chips, and gummy bear. All are to consume, and is a good place to stock up on food and Almond Water if you plan on progressing through the backrooms. You'll also find some magazines that contain stuff related to The Backrooms. On rare occasions, you'll find some lottery tickets on the floor. Scratching them will have the numbers 67482, 60012,and 43518. The reason for the same numbers is unknown. When you reach the end of the long isles, you'll come across 10 check-out counters all lined up. There's no employees or entities working on them though.

Lottery Tickets

Like mentioned before, you'll find lottery tickets throughout this level with the same three numbers when you scratch them. If you manage to collect 100 of these tickets, you'll find a door in the floor. Going through this door will lead to the pharmacy's basement.

Level 518's Basement

Once you're inside, the door in which you entered will disappear, meaning you can't get out through there. Upon entering, you'll be in a basement with rough walls and tile flooring. The basement stretches out for 300 miles. In this basement area are shelving which has several boxes, wooden planks, and some metal pipes. Inside these boxes are either medications or plastic pellets. The entire basement has most of the lights either flickering or out completely. With only a few actually working. When exploring this section of the level, you'll be bound to encounter hounds, skin-stealers and smilers. But in the basement, you'll find them posing as employees of the pharmacy, wanderers, or sometimes even dogs. Do not approach them, as you will be attacked. In some areas of this basement, you'll see wooden doors. If you enter these doors, you'll end up walking into Level 3 or Level 6. To exit the basement, you'll need to walk the 300 miles to the end. Once you reach the end, you'll see a trap door above you. Exiting through it will take you back to the beginning of the level.


Entities

While exploring this level, you're bound to come across 3 species of Entities. These are Smilers, Skin-Stealers and Hounds. The three of them on this level can be passive, aggressive or hostile. There's a moderate to high chance that they will appear on the isle you're walking down. If you see or hear them, run away in the opposite direction until their noises become faint, or until you can no longer see them.

Colonies and Outposts

Pharmacy Medical Department

This colony consists of 10 people who were once staff members of the pharmacy before it no clipped into the backrooms. These guys are open to trading, as well as providing medical aid.

M.E.G (Base Central)

Base Central is an outpost that was built in this pharmacy. The people who are stationed here consists of 32 Armed M.E.G Members, who were sent to explore, collect data, and try to exterminate the entities on Level 518. Base Central is also sometimes open to recruiting new members.


Entrances and Exits

Entrances:

- Fall asleep or pass out in Level 35

- Find a seemingly random wooden door in Level 117 and walk through it

- Walk through a metallic door labeled ‘Pharmacy’ Level 36

- Follow the instructions for escaping in Level 517

Exits:

- Reach the end of the isles, then walk through the exits. This will lead to Level 11

- Noclip through a purple shelf to enter Level 911 or Level 40, however the chances of the purple shelf appearing is rare.

- Go through the wooden doors in the basement, which will take you Levels 3 or 6

- Noclip through the ceiling to enter Level 519
