Level Pain spawns wanderers on an island surrounded by a vast sea of liquid pain.
Pain Island

Pain Island is the coined term for the spawn island at Level Pain. Pain Island is rocky with little to no flat land. The island’s biome prevents it from having arable soil. The bigger rocks on the island seep out a blood looking substance when broken.
The Gelatin Sea
The Gelatin Sea is the coined term for the sea at Level Pain. The liquid pain in this ocean is the same as liquid pain in other levels.
Do not ever swim in the ocean! Not only would the gelatinous consistency make it extremely hard to swim, liquid pain’s corrosive properties will slowly erode your body until you’re only molecules in the sea. Liquid pain can also be easily absorbed into the bloodstream, causing the same effects as consumption.
Freezing Point
Freezing Point is the coined term for the white island at Level Pain. The island is surrounded by hooks placed by the residents of Level Pain to deter wanderers from going there. Freezing Point is the only island on the level to feel extremely cold. The edges of the island measure at 32°F or 0°C. The further to the center one goes, the colder it gets. The coldest the island can get is -48°F or -44°C.
Wanderers theorize that the cold weather of the island is caused by something under the surface of the island, making the center as cold as it is.
Mount Pain
Mount Pain is the coined term for the mountain at Level Pain. The mountain takes up its entire island, meaning the island itself is also called Mount Pain. The mountain is mostly rocky, but covered with dirt at some points.
Entities
The entities in Level Pain are level-exclusive entities called Pain Eaters. Pain Eaters generally live on the floor of The Gelatin Sea. When Pain Eaters are hungry, they travel to the surface of Pain Island. The blood-like substance inside of rocks are their food. If the rocks aren’t already broken, the Pain Eaters will use their claws to scratch the rocks until they ‘bleed.’ The ‘blood’ of the now broken rocks are eaten until the entity is full. After feasting, they go back to the Gelatin Sea.
Their normal behavior changes when they see any organic animal including humans. Instead of feasting on the rocks, they’ll go up to the animal and scratch them instead. If a wanderer gets fed on by the Pain Eaters, they’ll have giant bleeding cuts on their body. If the cuts aren’t cleaned and dressed appropriately, the cuts will get infected. The infection has a high chance of leading to sepsis, which itself can be life threatening.
If you see a bleeding rock, go to the Pain Defense Team immediately.
How to Traverse The Gelatin Sea Safely
To traverse The Gelatin Sea safely, you need something to sail on that does not corrode from liquid pain. The rocks on the island are the only things on the island that not corrode from liquid pain. You can use these rocks to construct a boat.
The easier way to traverse The Gelatin Sea safely is to just ask the Pain Farmers if you can get a ride. The Pain Farmers usually oblige.
Colonies and Outposts
There are two known colonies on Pain Island.
Pain Defense Team
The Pain Defense Team is a small sub-section of the United Backrooms Authority with 10 members. This team shoots down Pain Eaters to protect the Pain Farmers and wanderers. 8 members of the defense team walk around to find broken rocks while the other 2 maintain the outpost.
Pain Farmers
The Pain Farmers consist of 24 farmers, 2 sails-people, and 2 traders. The Pain Farmers live on this level to collect the liquid pain to sell for profit. They are generally friendly to normal wanderers and are open to trade some of their supplies for more liquid pain.
